For instance, the section on calcaneal fractures refers to the angle of Gissane and Bohler without a description of where they are located on the bone. Osteoporose - Strategies de Prevention et de Traitement (I.N.S.E.R.M.; FRENCH
Abstract: L'ostéoporose est une affection caractérisée par une fragilité osseuse. Elle constitue un problème de santé publique important lié aux fractures qui viennent la compliquer. La fracture de l'extrémité supérieure du fémur, qui entraîne souvent une entrée en dépendance, et la fracture vertébrale altèrent sensiblement la qualité de vie. Sur la base des données épidémiologiques disponibles, il apparaît que l'ostéoporose touche environ 30 à 40 % des femmes ménopausées (soit au moins 3 millions de Françaises en 1995) et qu'après 75 ans, plus de la moitié de la population féminine en est atteinte. Même si elle reste une pathologie à dominante féminine, elle touche également les hommes au-delà de 75 ans. Cependant, les données cliniques et épidémiologiques font encore défaut sur ce point. Indépendamment de l'allongement de l'espérance de vie, on observe une augmentation régulière de l'incidence de l'ostéoporose, dans les pays industrialisés. A partir des projections démographiques, on peut estimer que le nombre de personnes victimes d'une fracture de l'extrémité supérieure du fémur passera, en France, d'environ 50 000 par an actuellement (pour 9,6 millions de femmes de plus de 50 ans) à plus de 100 000 en 2050 (pour 16,6 millions de femmes de plus de 50 ans). Cette évolution devrait toucher l'ensemble des pays et des régions développés. L'épidémiologie des fractures ostéoporotiques montre, en effet, qu'il existe, pour l'Europe, un double gradient des fractures, nord-sud et est-ouest: elles sont plus fréquentes en Europe du nord que dans les pays méditerranéens et, pour une région donnée, plus nombreuses dans les zones continentales que dans les zones océaniques. De ce point de vue, la France se retrouve dans une position moyenne par rapport aux extrêmes que constituent les pays scandinaves d'un côté, et des pays comme le Portugal et la Grèce de l'autre. L'ostéoporose, pathologie étroitement liée au cycle de vie du squelette En tant que pathologie dégénérative du tissu osseux, l'ostéoporose a partie liée avec le cycle de vie du squelette Loin d'être figé, le squelette est en effet une structure en perpétuelle évolution, marquée par un processus dynamique de renouvellement permanent des tissus qui le constituent. Ce processus est couramment désigné sous le terme de remodelage osseux. La résultante entre les quantités d'os formé et d'os résorbé constitue la balance osseuse, qui détermine, à son tour, les trois périodes du cycle de vie de l os. La période d'acquisition du capital osseux est caractérisée par une augmentation rapide de la masse osseuse. Elle correspond, chez la femme comme chez l'homme, à la période s'étendant de la naissance au seuil de la maturité (O à 30 ans environ). Il faut noter que la période de croissance pré-pubertaire est, en moyenne, de deux ans plus longue chez le garçon que chez la fille, de sorte que, à l'âge adulte, le pic de masse osseuse est plus élevé chez l'homme que chez la femme. La période de stabilisation du capital osseux correspond à la période de la maturité, jusqu'à la survenue de la ménopause chez la femme. La période suivant la ménopause chez la femme, et après 70 ans environ chez l'homme, est marquée par une perte osseuse significative tant au niveau de l'os cortical (diaphyse des os longs et enveloppe osseuse de tous les os) qu'au niveau de l'os trabéculaire (métaphyse des os longs et corps vertébraux) Cette perte osseuse conduit à la diminution de la densité osseuse, à l'augmentation de la porosité de l'os cortical ainsi qu'à la disparition des travées osseuses horizontales, aboutissant à la déstabilisation des travées osseuses verticales, à l'amincissement de ces travées et à la perforation osseuse. L'ostéoporose qui en résulte s'accompagne non seulement d'une diminution de la masse osseuse mais aussi de modifications de la micro-architecture osseuse, pouvant conduire à la fracture. En résumé, l'ostéoporose est une pathologie étroitement liée au cycle de vie de l'os. Pour reprendre la définition agréée par l'OMS en 1992, “ L'ostéoporose est une maladie caractérisée par une faible masse osseuse et la détérioration micro-architecturale du tissu osseux, une fragilité osseuse et, par suite, une augmentation du risque de fracture. ”... Assessment: This is ideal for new, in training physicians and medical students and those with limited exposure to athletes. Nous navons pas voulu être exhaustifs; ce qui eut été de toute manière impossible vu l'important développement des scores et classifications de ces dernières décennies. Chaque fois quune nouvelle technique apparaît et simplifie les traitements, des pans entiers de classifications deviennent obsolètes. Un devoir dhistorien persiste donc, qui doit permettre à chacun, selon le cas considéré, d'utiliser, soit des classifications « up to date », soit de rester dans des catégories plus classiques. Le premier tome est consacré aux classifications du rachis, de la hanche et du genou. Nous préparons le second tome qui sera, lui, consacré à lépaule, au membre supérieur et au pied. Ces livres se trouveront avec bonheur entre les mains aussi bien des plus chevronnés qui peuvent parfois avoir une hésitation et vouloir vérifier un point particulier , quentre celles de nos jeunes collègues, qui sont parfois victimes dune forme de pensée unique et souhaitent découvrir dautres horizons.
